Step 1: 

- Enter your name, address, and phone number.

- Register the email of a minimum of 2 - 4 players.

- Select the donation option(s) you would like to make.

- Add together your individual registration fees 
(dinner & golfing + additional dinners + any donation options you chose = TOTAL DONATION).

- Click the SUBMIT button to send your Registration Sign Up Form.


Step 2:

- Click on the donation button below to make your donation payment. 

- Enter your donation total.

Please fill out and submit the registration below.

(NOTE: There is a $90 golfer fee for each registered golfer which includes dinner, 18 holes of golf, and other amenities listed above. Additional dinners are available for non-golfers for $30 each).  Make sure you know a total dollars donated before you click submit. After you submit the form, click the donate button below the form to make your donation payment.
